Your Health
Medical Coverage
Employees can select a PPO or an HMO medical plan that best meets the needs of themselves and their families. Coverage becomes effective the first of the month following the date of regular employment.
Prescription Drug Plans
A prescription drug program is included when employees enroll in the Alpine Biomed medical plan. The prescription drug benefits offer the flexibility to purchase medications either at a local retail pharmacy or through mail-order service.
Vision Coverage
A vision care program is also included when employees enroll in the medical plan.
Dental Coverage
Alpine Biomed offers a comprehensive dental plan so that employees and their families can better manage dental costs.
Flexible Spending Accounts
Health Care and Dependent Care Flexible Spending Accounts are available to set aside pre-tax income for reimbursement of applicable daycare and health care expenses not otherwise covered by insurance. Up to $5,000 can be set aside to each account annually.
Life Insurance
Alpine Biomed provides life insurance at no cost to regular employees equal to their annual base salary. Employees can further increase this benefit by electing additional coverage. The cost for the additional level of coverage is paid by the employee. Evidence of insurability is required for certain coverage levels.
Accidental Death and Dismemberment Insurance
Alpine Biomed provides accidental death and dismemberment insurance at no cost to regular employees equal to their annual base salary. This insurance provides benefits in the event of injury or death as the result of accidental injury. Employees can further increase this benefit by electing additional coverage. The cost for the additional level of coverage is paid by the employee. Evidence of insurability is required for certain coverage levels.
Dependent Life Insurance
This optional employee paid coverage provides a death benefit in the event a covered family member dies. The cost depends on the coverage level elected. Evidence of insurability is required for certain coverage levels.
Dependent Accidental Death and Dismemberment Insurance
This optional employee paid benefit provides benefits in the event of injury or death as the result of accidental injury. Coverage is available for the employee’s spouse or domestic partner. The cost depends on the coverage level elected. Evidence of insurability is required for certain coverage levels.
Short-Term Disability
Disability benefits are provided in the event of a non work-related disabling illness or injury.
Long-Term Disability
This benefit is available to regular employees and provides income protection after 12 months of approved disability.
